9 Goals and Measurement (adoption of clinical pathway; efficacy outcomes measurement)

Level 1 Initial Inconsistent

  • Focus only on the Adoption process.

  • Inconsistent or no outcomes measurement

Level 2 Repeatable stabilized

  • Targeted organizational goals for deployment are established

  • Adjustment of work unit processes based on measures

  • Measure outcomes relevant to the work unit

Level 3-Defined Standardized

  • Defined measures (e.g. Key Performance indicators or Objectives and Key Results) of standardized adoption processes and reporting across the organization

  • Define standard outcome measures per clinical pathway

  • Adoption of quality measures, goals and quality reporting tools at an institutional level

Level 4 Quantitatively Managed

  • Integration of consistent tooling and processes (e.g. dashboards) across the enterprises to support consistent measurement and improvement adoption processes and pathway efficacy.

  • Process metrics are used to effectively control the process as well as outcome

  • Consistent review of efficacy of processes in impacting goals: Routine adjustment of process to advance outcome (moved from level 5)

  • Evaluate and continuously improve institutional capabilities informed by emerging industry best-practices

  • Measure efficacy of pilots experiments of new adoption processes and adjust accordingly

  • Conduct pilots and experiments of new adoption processes developed based on data (Moved to section 4 Process)

  • Continuous improvement and feedback look through routine data collection and submission to guideline owners (moved from level 4)
