Wound exudate amount reference set

Purpose

Provide values for recording and retrieving clinical observations / findings / ‘uber-observations’ that come about from a clinician examining a patient with a wound.

Reference – Bates-Jenson Wound Assessment Tool

Excerpts from Reference

Exudate Amount: Use a transparent metric measuring guide with concentric circles divided into 4 (25%) pie-shaped quadrants to determine percent of dressing involved with exudate. Use this guide:

None = wound tissues dry.

Scant = wound tissues moist; no measurable exudate.

Small = wound tissues wet; moisture evenly distributed in wound; drainage involves < 25% dressing.

Moderate = wound tissues saturated; drainage may or may not be evenly distributed in wound; drainage involves > 25% to < 75% dressing.

Large = wound tissues bathed in fluid; drainage freely expressed; may or may not be evenly distributed in wound; drainage involves > 75% of dressing.

Examples of included content

“Wound exudate - None”

Examples of excluded content (counterexamples)

“Scant”, “Heavy, foul-smelling wound exudate”

Editorial guidelines to cover this domain

Qualitative content that references widely used scales and/or questionnaires should reference an authoritative source that provides unambiguous (e.g. quantitative) meanings for the content. If there are no such sources, then the content ideally should be somehow annotated to give unambiguous, definite meanings to the concepts.
