recording values like height and weight is that they can be recorded by verbal report which cannot be used reliably-a use case in oncology is that the height is used and as it rarely changes over time (once accurately recorded) should persist in the record so that it is not replaced by self reported or other inaccurate measure for the purpose of calculating chemotheraby dosing...

Right - you have to have a concept of 'volatility' of a tracked value. Height is normally not volatile; HbA1C is, but in a months time domain, while heart rate is in mins/seconds.
