Scenario Details: While e-mailing David, Terry notices a new chronic care alert in his inbox. Opening the alert, Terry realizes that he will be due for several healthcare maintenance items in the next 30 days. As a PPD converter, he has to take a screening survey once a year, but he must also obtain a retinopathy screen and a comprehensive lipid profile for his diabetes. He is also informed that the complexity of his medical situation qualifies him for a medical discount on his electrical bill. Terry opens the attached application form from San Diego Gas and Power, completes it, takes the online TB survey, and books an appointment in the retinopathy screening clinic after his next obesity support group meeting.
